Thrive Physical Medicine is proud to offer Affordable Care plans to their patients so that paying for their healthcare does not prohibit them from seeking care. Payment of the bill is part of your treatment process and the primary responsibility lies with the patient. At Thrive we have put together a series of options for patients to help them to be better able to assume those responsibilities. If you do not participate in a group health plan under a PPO (preferred provider organization) the following are accepted forms of payment:
FORMS
Cash – Our practice accepts in the form of cash, check, Visa and Mastercard. We offer cash discounts for our patients paying out of pocket and work diligently to create the appropriate payment plan based on each patient’s ability to pay.
HMO – Our Chiropractors are contracted with most major HMO plans. If you have an HMO plan with which we are contracted, you need a referral authorization from your primary care physician. If we have not received an authorization prior to your arrival at the office, we will attempt to contact your primary care physician and obtain one. If we have not received a referral from your primary care physician, you may elect to be rescheduled or we will collect payment in full for all services performed, using your HMO's payment schedule
Medicare – Our Medical and Chiropractic providers accept Medicare. If you are a Medicare Beneficiary, we will verify your benefits and file the appropriate claims on your behalf. Nothing will be collected at time of visit unless you receive extended services not covered by Medicare. In that case, our staff will review this with you ahead of time and explain or estimate your financial responsibility using Medicare's Advanced Beneficiary Notice (ABN). If any amounts due outside of Medicare benefits that are patient responsibility will be put into an affordable care plan that is agreed upon by the patient.
Care Credit - Our office participates with Care Credit. Please call to speak to a Patient Services Representative for the application and approval process.
